Quick answer
Read Vorgoth And The Moon in Cinder's Cradle to start Hearing Voices. Follow its objective to the tower beside the Battlegrounds and read Breach. That document connects this short quest to Vorgoth's Ruminations, the longer diary-and-treasure trail. Start and next quest, completion document.
Check the quest before travelling
Hearing Voices has one listed objective: Find Traces Of Lord Vorgoth. It is an exploration quest in Cinder's Cradle; the instruction is to investigate the tower, rather than collect a crafting material or defeat the final boss. If the name in your journal differs, check which diary quest you are tracking before setting out. Objective reference.
Locate the tower by the Battlegrounds
The Battlegrounds lies beyond the Ancient Gate in the lower part of Cinder's Cradle. The relevant tower stands beside that battlefield. The source description distinguishes this lower-area stop from the floating islands visited later in Vorgoth's Ruminations. Battlegrounds location.
Enshrouded.tools places the objective marker approximately 810 metres north of Drak'Dal Temple. This is a map reference, not a safe travel route or a claim that the temple is the best departure point. Use the tracked objective and terrain to find an approach you can actually traverse. Marker reference.
Search for Breach inside the tower
Search the tower for the first of Vorgoth's diary pages, Breach, and interact with it. Then inspect the journal. The next quest should concern finding further diary pages; remaining in the tower after the quest changes will not move that later search forward. The available quest data connects the two quests, while the Battlegrounds description identifies the first diary page at the tower. Diary connection.

Do I need to fight Vorgoth here?
The listed Hearing Voices objective is to find traces of him. The sources used here do not list defeating him as this quest's completion condition.
Why am I now being asked for another note?
You may have moved into Vorgoth's Ruminations. Check the quest title and objective; the later trail involves the remaining diary pages and a buried chest.
What is the reward?
The sources agree on 225 XP. Item-reward descriptions differ across databases, so no specific item is promised here. The MaxDPS record shows a silver reward-chest icon without fixed contents. Reward details.
